EMT Test

The National Registry of Emergency Medical Technicians (NREMT) administers a cognitive exam that all aspiring EMTs must pass to earn their license.

This exam is a Computer Adaptive Test (CAT) ranging from 70 to 120 questions, with a 2-hour time limit. Ten of these are “pilot” questions—unscored items used to refine future exams. Since you won’t know which questions are unscored, it’s critical to tackle every question with your best effort.

The test splits into adult (85%) and pediatric (15%) content across most sections, except for EMS Operations. Here’s the breakdown of topics:

  • Airway, Respiration & Ventilation: 18–22% of questions
  • Cardiology & Resuscitation: 20–24% of questions
  • Trauma: 14–18% of questions
  • Medical/Obstetrics/Gynecology: 27–31% of questions
  • EMS Operations: 10–14% of questions

Note: Certification also requires passing a psychomotor exam, a hands-on skills test not managed by the NREMT. This is typically provided by your training program or State EMS Office.

Is having 70 questions on the NREMT a good or bad sign?

The NREMT (National Registry of Emergency Medical Technicians) exam is computer-adaptive, meaning the number of questions (typically 70–120) depends on your performance. Getting 70 questions isn’t inherently good or bad—it just means the test determined your competency early. If you’re consistently answering correctly, it might stop at 70 because you’ve proven proficiency. If you’re struggling, it could stop because you’ve hit the minimum threshold without passing. The key is how well you did, not the exact number.

How challenging is the NREMT exam?

The NREMT is considered moderately to highly difficult, depending on your preparation and experience. It tests critical thinking under pressure, not just rote memorization, with scenarios covering medical emergencies, trauma, and EMS operations. Pass rates vary (around 70% for first-time takers), but many find it tough due to its adaptive nature and time limit (about 2 hours).

What’s the most effective way to prepare for the NREMT exam?

To prepare well:

  • Study the basics: Focus on the NREMT’s content outline (airway, breathing, circulation, trauma, etc.).
  • Practice scenarios: Use practice exams to get comfortable with the question style and pacing.
  • Hands-on review: Reinforce skills like patient assessment or CPR if you’re rusty.
  • Time management: Simulate the test environment to build stamina.
  • Review weak spots: After practice tests, target areas where you score low. Consistency over cramming is key.
